Existing Vacancy: Yes – we’re currently hiring candidates for an existing vacancy in this position  Job Summary:  The Case Coordinator provides clients with comprehensive assessments ensuring a high-quality client experience. The Case Coordinator is responsible for developing, in consultation with the client and caregiver, a comprehensive care plan to meet the client’s goals and for supporting the client and caregiver to manage their care budgets and required resources. The Case Coordinator supports the philosophy of client self-management and works within the framework of VON policies, procedures, standards and quality & risk management. The Coordinator is responsible for developing and supporting an individual support plan for clients with developmental disabilities in receipt of residential support from the Family Home Program. These services are funded by the Ministry of Children, Community and Social Services. The Coordinator reviews and assesses the needs of individuals requiring support related to all aspects of their support plan. The coordinator is responsible for providing a high quality, successful residential service for clients in our community. Key Responsibilities: Common Responsibilities:External and Internal Relationships:Education, Designations and Experience:Skill Requirements: Other: Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
